Dylai’r Llywodraeth: (a) Taclo achosion gwraidd marwoldeb babanod a phlant, yn cynnwys amddifadedd ac anghydraddoldeb. (b) Cyflawni gwiriadau awtomatig, annibynnol a chyhoeddus o farwolaethau neu anafiadau difrifol annisgwyl i blant yn y ddalfa, mewn sefydliadau gofal ac iechyd meddwl ar draws y Deyrnas Unedig.
Argymhelliad gwreiddiol y CU
The Committee recommends that the State party: (a) Address underlying determinants of infant and child mortality, including social and economic deprivation and inequality; (b) Introduce automatic, independent and public reviews of unexpected death or serious injury involving children, including in custody, care and mental health-care institutions in all the territory of the State party.
